In addition to the ten main activities, there are three questionnaires to complete when assessing a learner in SNAP Maths:
- Teacher questionnaire: a multiple-choice questionnaire about the learner's maths skills and their emotional response to maths. It should be completed by a staff member who knows them best.
- Parent/carer questionnaire: collects information about the learner's maths skills and their emotional response to maths as observed by their parent(s) or carer(s).
- Pupil questionnaire: gives insight into the learner's own perceptions about their maths ability and their feelings about maths.
The questionnaires are optional, but we strongly recommend that you complete them to create a holistic picture of the learner's abilities.
Accessing the questionnaires
You can find the questionnaires at the bottom of the 'Assess' tab. Each questionnaire can be downloaded as a PDF, but the answers must still be entered into the platform using Open questionnaire buttons.
The Pupil questionnaire can also be accessed via the activities URL:
- Click the Resource bank tab.
- Find the activities URL and unique centre code in the 'Taking the activities' panel.
- Copy and paste the URL and code somewhere the learner can access it.
When the learner accesses the activities URL, they'll be asked to enter the unique centre code, followed by their date of birth. The questionnaire will be available on the activities selection screen.
Completing the questionnaires
Both questionnaires open in a new window. If they don't open, check the pop-up settings for your browser.
When you select an answer, the screen will automatically move to the next question. You can amend an answer at any time by clicking Back or Next at the bottom of the screen.
Progress is saved automatically, so you don't need to complete a questionnaire all at once. If you close the window, then re-open it, it will resume from where you left off.
On the Teacher questionnaire, if you are unsure of an answer, click Don’t know. Please note that this does not create a score like the other answers. If you select this answer every question in a section, no result will appear on the Questionnaire profile.
On the Pupil questionnaire, click the speaker icon in the top right to have the questions and answer options read aloud.
The Open questionnaire buttons on the 'Assess' tab will turn green as the questionnaires are completed:
